Tufts Health One Care brings your MassHealth (Medicaid) and Medicare benefits together. You’ll have one health plan, one insurance card and one person – a care coordinator – who helps you get the care you need. And you’ll pay no deductible, no monthly premiums and no copayments.

Eligibility
You may be eligible9 for Tufts Health One Care if you:
- Are 21 to 64 years old
- Have MassHealth Standard or MassHealth CommonHealth and Medicare
- Live in one of these counties: Barnstable, Bristol, Essex, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk, and Worcester.10
People are ineligible if they have private health insurance from their job or a home and community-based services waiver.

1 You are not eligible to enroll into Tufts Health One Care if you are enrolled in any other health insurance plan, with the exception of Medicare. Other eligibility requirements and restrictions may apply .
2 Non-emergency. Non-medical rides limited to 8 round trips per month, 20 miles each way of trip. Members must use plan-approved vendor to access benefit. Exceptions may apply.
3 $300 available toward the full retail price (not sale price) for eyeglasses (lenses, frames, or a combination) and/or contact lenses from any provider.
4 Quantity limit applies. Covers one hearing aid per ear per member every 60 months.
5 Please call 911 for life-threatening emergencies. In-home urgent care services are provided where permitted by law and, as required, facilitated by and/or with advanced practice practitioners and/or physicians.
6 Flexible Benefits available based on inclusion in members Individualized Care Plan. Please discuss this with your care coordinator.
7 Benefit and/or network limits may apply. Services must be performed by a DentaQuest provider.
8$155/calendar quarter credit is for the purchase of eligible plan-approved OTC items as well as eligible groceries from participating retailers and plan-approved online stores. Unused balance at the end of a calendar quarter does not roll over. Under certain circumstances, items may be covered under your Medicare Part B or Part D benefit.
9 YMCA membership is valid at a local YMCA facility located within our service area in Massachusetts.
10 Not available in Berkshire, Dukes, Franklin, or Nantucket Counties.
Tufts Health One Care is an D-SNP with a Medicaid and Medicare contract. Enrollment in Tufts Health One Care depends on contract renewal.
